All medication must be clearly labelled with a pharmacy label containing the campers full name, dosage, doctors name and expiry date.

We ask that all medication be held and administered by the participant where possible and if they require a designated leader for assistance to please contact our youth team before camp.

Financial Assistance

INDEMNITY FORM

I authorise the leader in charge of the NCAYouth Winter Attack Camp 2026, where it is impracticable to communicate with me, to arrange for my child to receive such medical treatment as the leader may deem necessary at any time during the time away. I further authorise the use of an Ambulance if in the leaders judgment it is necessary. I accept responsibility for payment of all expenses associated with such treatment.

I give full permission for the above mentioned camper/s to participate fully in all activities associated with NCAYouth Winter Attack Camp 2026. While every precaution shall be taken to ensure the good welfare and protection of the applicant camper/s, Naremburn Cammeray Anglican Church or any leader acting on their behalf are hereby released from any and all liability in the event of any accident or misfortune that may occur to the applicant camper/s or damage or loss to their property.

My child agrees to follow the rules and directions given by the leaders. I agree to my child attending the NCAYouth Winter Attack Camp 2026 on this understanding.
