I authorise the leader in charge of the Christmas Workshop held at Naremburn Anglican Church where it is impracticable to communicate with me, to arrange for my child to receive such medical treatment as the leader may deem necessary at any time during the day. I further authorise the use of Ambulance if in the leader’s judgment it is necessary. I accept responsibility for payment of all expenses associated with such treatment.

I appreciate that the leaders will take all reasonable care for the safety of my child. I also understand that the leaders cannot be held responsible for personal injury, loss or theft of property affecting my child.

My child agrees to follow the rules and directions given by the leaders. I agree to my child attending the Christmas Workshop on this understanding.

I understand that my child should only attend Christmas Workshop if they are feeling well and not displaying symptoms of cold, flu, Covid-19 or any other illnesses.
